1. RELEASE. I hereby grant permission to Change Lab, LLC dba Over Everything Else (“OEE”) and its Affiliates, the rights of my image, in video or still, and of the likeness and sound of my voice as recorded on audio or video tape. I understand that my image may be saved, edited, copied, exhibited, published or distributed and transcribed in written form at the producer's discretion and waive the right to inspect or approve the finished product wherein my likeness or transcribed statements appear.

2. OWNERSHIP AND USE. OEE and its Affiliates have complete ownership of such material, and can use said material for any purpose consistent with the Project and OEE’s mission. These uses include, but are not limited to photographs, videos, audio online content and website, educational materials, written publications and resources in any medium. By signing this release, I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the internet.

3. TIME AND LOCATION. This release shall remain binding in perpetuity and scope of which has no geographic limitation on where these materials may be distributed.

4. COMPENSATION. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording or transcribed statements. 

5. CONFIDENTIALITY. I acknowledge that I may have access to confidential, proprietary, and/or trade secret information belonging to the OEE. I agree that you will keep all of this information strictly confidential and refrain from using it for your own purposes or from disclosing it to anyone outside those individuals or entities expressly authorized by OEE.

6. RELEASE: I release OEE, its employees, contractors, agents, assigns, and/or third parties associated with filming locations from all liability which may arise from any and/or all claims by me or any third party in connection with my participation in the Production. I agree to pay for damages to any and all items, property, and/or equipment related to the Production that results from my negligent and/or reckless behavior. 

7. ACCIDENT WAIVER. I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: 

   1. WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity, the Company and/or its directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers; 

   2. INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise. 

8. RELEASING PARTIES These agreed to releases apply to me, my heirs, next of kin, spouse or spousal equivalent, guardians, legal representatives, executors, administrations, successors and assigns. I and the above parties hereby voluntarily and knowingly release, discharge, and relinquish any and all claims, action and lawsuits of any kind against the OEE, or causes of action that I may have or may later have for libel, defamation, invasion of privacy or right of publicity, infringement of copyright or violation of any other right arising out of or relating to any utilization of the rights granted herein. 

By agreeing to this release, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release and discharge any and all liability or legal claims against any person or organization utilizing this material.
